  ​Community Habilitation provides individualized supports for a person to become more independent and gain skills that will allow him/her to functions in the community. This habilitation service facilitated community inclusion, integration, and relationship building. This space is ideal for writing a detailed description of your business and the types of services that you provide. 

Program Activities

Community Habilitation program activities vary, and are dependent on a number of factors, which includes the interest of the Individual and what has been determined in the Habilitation plan.

As our services are community based, we incorporate integration into the community with many of the activities. It is very important for the Individual to be familiar with the many resources that the various communities offer. With support from our staff, activities that take place in the community are meaningful and viable. There are a number of activities that can take place within the home as well, however they should be relative to the valued outcome.

Valued Outcome

A Valued Outcome is the overall ”goal” that the Individual would like to achieve. It is what the person would like to do or accomplish for themselves. The Valued Outcome can change based on the Individual. There are activities that can be done in order for the Individual to reach his or her Valued Outcome. The activities can change once the person has accomplished them, if the person no longer wants to work on the specific activity or the Individual does not make progress with the activity in place.
